The current LIC 602A PDF sits on the state forms list under I-L. The entry there reads LIC 602A (4/26), and that stamp in brackets is the whole test.
Everything else follows from comparing two stamps. Find the one printed on your copy, then look up the one shown beside LIC 602A on the California Department of Social Services alphabetic forms list . If they match, the copy is current. If they do not, it is an old revision.
Old revisions are easy to pick up by accident. Search the form name and the results still include copies hosted on facility websites, county pages, and commercial form sites, some of them stamped 12/04 or 8/11. A facility that checks the stamp will send that copy back.
Where Does the Current LIC 602A PDF Actually Live?
On the state agency's own forms list, not on a facility site or a form reseller. CDSS keeps every community care licensing form in alphabetic lists, and because the form number starts with L, the LIC forms sit in the I-L section.
The Alphabetic Forms List
The list shows the form number, the revision stamp, and the official title side by side, which is why it answers the version question faster than the PDF does. CDSS also keeps the forms organized by program and in translated versions, all from the same hub.
Download It Rather Than Filling It In the Browser
CDSS says plainly that its forms should be downloaded, because browser constraints can stop a fillable PDF from working properly. Save the file first, then open it in a PDF reader. Typing into a form inside a browser tab is how people lose an hour of entries.
How Do You Read the Revision Stamp?
The stamp is the month and year the form was last revised, written as digits in brackets. LIC 602A (4/26) means a revision from April 2026. It is printed on the form itself, usually in the corner next to the form number, and it repeats on each page.
Two things are worth knowing about the convention. The stamp tracks the form, not the resident, so it never changes because of who filled it in. And a newer stamp does not make an older signed form invalid on its own, which is a separate question covered further down.
The word CONFIDENTIAL often sits beside the stamp, along with a page count such as PAGE 1 OF 6. A copy whose page count does not match the current file is another sign you are holding something older than you think.
How Can You Tell an Outdated Copy of the 602 Form PDF?
Compare the stamp with the CDSS list and nothing else. Not the date in the file name, not the date the website says it was uploaded, and not the date your browser shows for the download. Only the stamp printed on the form matters.
Stamps That Look Old but Are Still Current
This is the part that trips people up. An old-looking stamp is not automatically wrong, because some of these forms simply have not been revised in years. The CDSS list is the only thing that settles it, and the four forms families meet most often are set out below.
| Form | Stamp on the CDSS list | Official title |
|---|---|---|
| LIC 602 | (7/22) | Physician's Report For Community Care Facilities |
| LIC 602A | (4/26) | Medical Assessment for Residential Care Facilities for the Elderly |
| LIC 603 | (6/22) | Preplacement Appraisal Information |
| LIC 603A | (7/99) | Resident Appraisal |
LIC 603A is the clearest example. Its stamp reads 7/99, which looks like a mistake until you check the list and find that it really is the current version. Judging a form by how dated the stamp looks produces the wrong answer in both directions.
Where Stale Copies Come From
Three sources account for most of them. Facility admission packets that were assembled years ago and photocopied since, county and agency pages that posted a copy once and never refreshed it, and commercial form sites that rank well in search and update whenever they feel like it.
A doctor's office can be a fourth. A clinic that keeps a stack of blank forms in a drawer may hand over a revision from two cycles back without anyone noticing, so it is worth checking the stamp before the exam rather than after.
Is the LIC 602 PDF the Same as the LIC 602A?
No, and mixing them up is more common than using an outdated version. LIC 602 is the Physician's Report For Community Care Facilities. LIC 602A is the Medical Assessment for Residential Care Facilities for the Elderly, and an assisted living move-in needs that one.
The titles explain the split. Community care facilities cover a wider set of licensed programs, while an RCFE is the elderly-specific license, and each has its own report. Which form a given facility is asking for is worth settling before the appointment (LIC 602 vs. LIC 602A: Which Physician's Report a Facility Is Actually Asking For).
The practical shortcut is to ask the facility for the form number and the stamp in one message. Any admissions coordinator can read both off their own packet, and that single reply removes the guesswork from the download.
Does an Outdated Form Get Rejected?
Usually, the reason is the resident record rather than the form design. California regulations require the facility to obtain documentation of a medical assessment signed by a licensed medical professional acting within the scope of their practice and made within the last year, and to keep it in the resident's record (Cal. Code Regs. Tit. 22, § 87458 - Medical Assessment).
That rule was amended on October 17, 2024 and became operative on January 1, 2025, which is part of why versions from the preceding years are still floating around. A facility working to the current rule wants the current paperwork to match.
Licensing is the reason the facility cares. The Community Care Licensing Division runs the regulatory enforcement system for senior care and the other licensed program areas, and its analysts review resident records during visits . An incomplete or superseded form in a file is the facility's problem, not the family's, which is why admissions staff check it closely.
What this does not mean is that a signed form stops counting the moment a new revision appears. The one-year window is about when the assessment was made. Validity and renewal are their own subjects (LIC 602A After the Hospital: How to Get the Form Fast for an RCFE).
Which Other CCLD Forms Come With It?
At least one more, and often several. The medical assessment is the clinical half of the admission file, and the facility completes a separate appraisal of its own before anyone moves in.
Regulations require a pre-admission appraisal documenting functional capabilities, mental condition, and social factors, developed with the prospective resident and their responsible person, and conducted by the licensee or the employee responsible for admissions (Cal. Code Regs. Tit. 22, § 87457 - Pre-Admission Appraisal). That is the LIC 603 side of the packet, and it is not a doctor's job.
Knowing which signature belongs on which form saves a second trip to the clinic. The division of labor between the two appraisals is set out separately (LIC 602A and LIC 603: Two Appraisals, Two Signers), and the admission agreement that arrives alongside them is a third document worth reading before signing (The Admission Agreement an RCFE Hands You Alongside the LIC 602A).
How Do You Get the Current Form Completed?
Download the current PDF first, then book the exam, then handle the tuberculosis line last. Running those three steps in that order avoids the most common delay, which is a completed form waiting on one empty field.
The medical part needs a licensed provider, and getting to a clinic is the hard step when someone is frail or recovering. A doctor who visits at home can examine the person and complete the report in the same visit, and a form request can be started with the facility's form number and stamp in hand.
The tuberculosis result is the field that holds everything up, because it depends on test timing rather than on the appointment. Which screening method fits, and how the result reaches the form, is covered in detail elsewhere (TB Test for LIC 602A: Choosing Between Skin and Blood Screening for RCFE Admission).
FAQ
Is there a blank 602 form that can be filled in by hand?
Yes. The CDSS file is a PDF that can be printed and completed on paper, or downloaded and filled in on a computer. Printing a blank copy is fine as long as the stamp matches the current entry on the forms list.
Does CDSS publish the form in languages other than English?
CDSS keeps a translated forms and publications section alongside the alphabetic lists. Which forms appear there varies, so check that section directly rather than assuming a translation exists for a given form number.
Can a facility insist on its own version of the packet?
A facility can add its own documents, and many do. What it cannot do is substitute a different document for the state form the regulations require, so the LIC form still has to be the CDSS one.
What if the form number on the facility's request does not exist on the list?
Ask the facility to re-read it. Form numbers get transcribed wrong, and a digit dropped from a number turns into a form for a completely different licensed program. The CDSS list is the reference for settling it.
Where do the other LIC forms sit on the site?
All of them are in the same alphabetic lists, split by first letter, with a parallel set organized by program. A form number beginning with LIC will always be in the I-L section.
Does the revision stamp change when the state updates the regulations?
Not automatically. Rules and forms are revised on separate schedules, so a regulation can change without a new form appearing, and a form can be reissued without any rule changing. Check the list rather than inferring one from the other.
